PPFE：上葉優位型肺線維症/私が唯一自信があること

・以前この疾患をとりあげたかどうか既に記憶にございませんが、たとえ取り上げていたとしても記憶の強化のため本日は、このお話。

・標題の上葉優位型肺線維症は、希な疾患で私はほとんど経験したことはありません。ただ、私が若かりし頃レントゲン読影をおしえてもらっていた「お師匠」さまに、間質性肺炎でも１割くらい（だったかな？）上葉優位の物がありますといわれて、へー、そうなんだと思ったことは覚えております。で、以前は、（今も？）希な疾患で「知名度」も低かったでしょうが、最近の「新しい」分類で、希な特発性肺線維症にiPPFEが入れられて少しはゆうめいになったでしょうか。↓のような論文がありました。

【要旨】 網谷らは1992年，上葉に限局した肺線維症で，結核などの原因の明らかでない13例を，特発性上葉限局型肺線維症（idiopathic pulmonary upper lobe fibrosis: IPUF）として報告した．IPUFは，胸郭は極めて扁平，胸郭外病変はなく，緩徐で はあるが確実に進行し，10–20年の経過で死亡する例が多い．病理学的には，両肺上葉が著しく縮小化し，両側肺門が挙上． 線維化病変は，上肺野に限局してみられ，下葉にはほとんど病変は認められない．胸膜の線維性肥厚があり，気腔内を充 満する形の線維化が形成され，時間経過した部分では，肺胞が虚脱，肺胞壁は折り畳まれた形となり，弾性線維が一見増 加したように観察される．線維化巣と下方の正常肺の境界は明瞭である．近年，idiopathic pleuroparenchymal fibroelastosis（idiopathic PPFE）としてIPUF類似の組織像を呈する上葉優位型の病変が報告され，新しい間質性肺炎の組織学的分 類のなかに，rare IIPsの一つとして取り上げられるようで，さらなる知見の蓄積がなされるものと期待したい．

　特発性間質性肺炎（idiopathic interstitial pneumonias：IIPs）は，原因や基礎疾患を特定できない間質性肺炎 の総称である．現在，国際分類上，9つの病型に分けられる．そのなかで，患者数が最多である特発性肺線維症 （idiopathic pulmonary fibrosis：IPF）は，慢性経過をたどる予後不良の疾患であり，ステロイド治療が無効で ある点が他の病型と異なる．現在，抗線維化薬が導入されるようになり，予後の改善が期待されている．2014 年，特定疾患医療受給者であるIPF患者の疫学統計が明らかにされ，我が国では，有病率10万対10，生存中央値 が約3年，死因の40％が急性増悪であることが示された．なお，特定疾患医療給付制度に未登録の潜在的患者は その数倍存在すると推定されている．今後，2018年に改訂された国際ガイドラインに準拠した的確な診断のも と，早期からの治療介入が望まれる

・7/7にブログを書いて依頼久しぶりの記事のアップです。その時に映画の『最高の人生の見つけ方』を観たことがないと書きました。自分でもそう信じていたので翌日Amazon Prime Videoで観るようにしたのですが、みていて、こりゃー以前観たわと思い出しました。ただ、同じ映画を観るのも良いし、同じ本を読むのも良いでしょう。時間がたつと全くおなじ者でも、違った物が見えてきて、違った感想をもてるので。しかし、それは、最初にみたり読んだ時の感想をおぼえていたら、ですが。で、観た感想ですが、なかなかうまくまとめられません。ただ、言えるのは自分だったらあんなに派手なことは死ぬ前にはしないでしょう。せいぜいフランスに行って美術館巡りをすることでしょう。あとは、我が家にあるまだよんでいなかった本を読むことでしょうかね。あとは、小さい頃けっこうあちこち転居していたので、もと住んででいたところを尋ねるでしょうか。・・・実はコロナが流行する前、それをやっておこうと思って、まずは京都の上桂にいきたかったのです。これは小学校入る前まで住んでいたので。しかし、コロナ流行で挫折。で、最近落ち着いてきたので行ってみようなかなと考えていたら、第７派？もう、やだっ！

・それは、さておき本日のブログの後半。私が唯一自信があることは何か、それは忘れること。最初に書いたようにブログに何をかいたか覚えていない。7/9（土）は宿直、7/10（日）は続けて日直で帰宅したのは、17時18分。当然（？？）その後アルコール飲んでAmazon Prime Videoで続けて二つ洋画を見たのですが、翌日最初に何を観たかをまったく覚えていないのです。もう、ビックリ、ポンポン。まあ、しばらくして思い出しましたが．．．ヤバイ、老・テンション。

・さて、ちょっとだけ過去を振り返ります。

・昨日7/11（月）は、6時起床。朝勉して、朝食摂って、燃えるゴミ出して午前中はZOOMで因果推論の勉強会。午後から出勤して回診して夜間診療。帰宅は19時40分。夕食、入浴、学位審査のスライド作成。

・本日7/12（火）は、5時37分起床。学位審査のスライド作って早めに出勤。病棟回診し、午前外来。昼食後ささっと病棟回って14時41分帰宅。Teamsの準備。15時からTeams

起動し、15時10分から15時40分くらいまで学位審査のプレゼンテイーション予行演習。20分持ち時間あるのに14分弱で終了。もうちょっとスライド増やしましょう。その後一服後今度の金曜日にする研修医向けの講義（共同の営みとして捉えた医学・医療）のパワポ作成。入浴、夕食。そしてアルコールいただいております。このブログアップしたら、歯磨きして早めに寝ます。ああ、大画面で映画みたい。
